Add refcounting to SerialPort
...
The library hands out pointers to SerialPort objects which are then simply
deleted on port removal. This fix makes the code port hot-unplug friendly.

Fix synchronization in PnPControl
...
Checking the number of ports must be done inside the critical section.
Also the inequality was incorrect, not to mention the missing LeaveCS
in PortSymbolicName().
